The front and back of your card is needed, also if there is a back to your ID with a signature that will be needed as well.  
While the Credit Card Authorization Request Form and process may seem excessive, DeepDiscount has adopted these protocols following an order placed for a high dollar amount for one reason only…to protect our customers. I regret that your experience has placed us in conflict and we would hate to lose a customer by turning down your business. The form will be needed no matter where you send the order.  As long as the back of your card has a signature that will be fine.
